This field trip is an enrichment experience for participating students. Submission of this completed form is mandatory for participation; no student will be permitted to join the trip without a signed authorization and release form.
Acknowledgement of Refund Policy:
- Deposits and Fees: All deposits and trip fees are non-refundable, regardless of changes in attendance or cancellation by the parent/guardian after payment.
- Behavior-Related Removal: Should a student receive OSS (Out-of-School Suspension), ISS (In-School Suspension), or is otherwise removed from the trip at the discretion of the principal, teacher, or school administration, no refunds will be issued for any deposits, payments, or associated costs.
- Other Absences: No refunds will be provided for school activities missed due to the trip, including but not limited to sports practices or games, catered lunch orders, other scheduled field trips, club activities, aftercare, or any additional extracurricular programs.
Insurance Requirement and Release of Liability: Each student must have personal insurance coverage to address any potential accidents or injuries sustained during the trip. Any third-party travel insurance option provided will be at the discretion of the parent/guardian to accept or decline, and Union Academy will not supplement any gaps in coverage if third-party insurance is declined. The responsibility for this decision rests solely with the parent/guardian.
Assumption of Risk: Participation in this trip involves inherent risks, including those associated with travel, transportation, activities, and interactions with persons not affiliated with the school. The school does not assume liability for any injury, illness, or harm occurring during any portion of the trip, including travel via commercial or privately chartered transportation.
Release of Liability: The undersigned parent/guardian acknowledges and voluntarily releases Union Academy Charter School and its agents from all liability for any injury, harm, damage, or loss incurred during any portion of the trip, including while traveling on buses and during the loading or unloading process at any location. This release applies to all claims arising out of the trip except those directly resulting from the school’s willful or wanton misconduct, as governed by the laws of North Carolina.
Medical Care Authorization: In the event of an accident, injury, or emergency involving the student, school staff will seek medical care or other necessary assistance as promptly as possible and will notify the parent/guardian immediately. The parent/guardian accepts full financial responsibility for any medical services provided.
- Authorization for Medical Care: By signing below, the parent/guardian authorizes Union Academy and its designated staff to secure necessary medical personnel, conduct routine or emergency procedures, release records necessary for health or insurance purposes, and administer any other required actions to protect the student's well-being.
Student Conduct and Behavioral Compliance: All students are required to adhere to Union Academy’s behavior policies and rules for the duration of the trip. School staff retain full authority to address noncompliance, which may include, but is not limited to, contacting the parent/guardian to arrange immediate pickup or alternative transportation, if needed, at the parent’s expense.
